The Medication Therapy Management—MTM—program is a quality improvement initiative required by the Centers for Medicare & Medicaid Services as part of the Medicare Modernization Act of 2003. The purpose of the program is to optimize therapeutic outcomes for targeted beneficiaries by improving medication use and reducing adverse drug events.
