The prostate is a small gland, approximately the size of a walnut that is located under the bladder and in front of the rectum and surrounds the first part of the urethra that is responsible for carrying urine from the bladder to the penis. The prostate gland is an exocrine gland that is responsible for producing some of the fluid that is expelled from the body during the male orgasm. Prostate cancer (PC) occurs when cells within the gland mutate and reproduce in an uncontrolled manner. PC may spread through the body through the blood supply or lymphatic system, resulting in metastasis. PC is the most common non-skin cancer in America affecting close to 1 in 6 men. Incidence of cancer is directly correlated with age, race, and family history. African American men are over 60% more likely to develop cancer in comparison to Caucasian men of the same age. In addition, men with three or more first degree relatives (father or brothers) diagnosed with PC, or three generations on either side of the family, or two family members diagnosed before the age of 55 may increase the risk of developing PC.
Screening for PC can be performed by a physician by performing a digital rectal exam (DRE) and a prostate specific antigen (PSA) blood test. Although a DRE and PSA test cannot diagnose cancer, they will indicate the need for a biopsy to confirm the presence of cancerous cells. Screening is recommended starting at the age of 50 except for those that are considered high risk, and therefore should begin testing around the age of 45. Although it is uncommon for men under the age of 50 to have PC, any of the following symptoms should result a follow-up with the patient's physician: frequent urination (especially at night), difficulty in starting to urinate, weak or interrupted flow while urinating, a burning sensation while urinating, difficulty in attaining an erection, pain during ejaculation, and blood in the urine or semen.
There are a variety of treatment options for PC that include prostatectomy, radiation therapy, hormone therapy, and chemotherapy to name a few. Due to the significant side effects that may arise as a result of treatment including, erectile dysfunction and urinary incontinence, the decision on treatment protocols must balance the goal of eliminating the cancer with the risks associates with lifestyle modifications as a result of treatment side effects.
